This stagnation can lead to blood clots forming within the appendage, which may subsequently escape into the bloodstream, causing peripheral embolisms, particularly ischemic strokes (Fig. 2).<\/p>\n\n\n\n<h3 class=\"kt-adv-heading191_07ae6d-2b wp-block-kadence-advancedheading\" data-kb-block=\"kb-adv-heading191_07ae6d-2b\"><strong>20% of all ischemic strokes are caused by atrial fibrillation.&nbsp;<\/strong><\/h3>\n\n\n\n<p>At-risk patients are usually prescribed anticoagulant medications to mitigate this risk.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter size-full\"><img decoding=\"async\" src=\"https:\/\/grossicardiologo.it\/wp-content\/uploads\/occlusione-percutanea-fig.2.png\" alt=\"occlusione percutanea\" class=\"wp-image-1019\"\/><figcaption class=\"wp-element-caption translation-block\">Fig. 2\nA: Thrombus formation (T) within the left atrial appendage (LAA).\nB, C: Progressive movement of the thrombus out of the appendage toward the left atrium (LA).\nD: The thrombus is no longer visible, having entered the bloodstream, significantly increasing the risk of stroke.<\/figcaption><\/figure>\n\n\n\n<p class=\"kt-adv-heading191_d4543f-22 wp-block-kadence-advancedheading\" data-kb-block=\"kb-adv-heading191_d4543f-22\">When anticoagulants are contraindicated, poorly tolerated, or ineffective for a patient, an alternative is to close the appendage to prevent blood from circulating within it, thus eliminating the risk of clot formation.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading has--font-size\">How is left atrial appendage occlusion performed?<\/h2>\n\n\n\n<p>The procedure is performed percutaneously via the blood vessels in the groin, similar to ablation procedures.\nPatients are generally sedated, and general anesthesia is typically unnecessary.\nOnce the left atrium is accessed, a device is positioned within the appendage to completely occlude its lumen, excluding it from blood circulation (Fig. 3A, 3B).<\/p>\n\n\n<div class=\"kb-row-layout-wrap kb-row-layout-id191_6b5ac0-ed alignnone wp-block-kadence-rowlayout\"><div class=\"kt-row-column-wrap kt-has-2-columns kt-row-layout-equal kt-tab-layout-inherit kt-mobile-layout-row kt-row-valign-top\">\n\n<div class=\"wp-block-kadence-column kadence-column191_27bdcd-db\"><div class=\"kt-inside-inner-col\">\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" src=\"https:\/\/grossicardiologo.it\/wp-content\/uploads\/occlusione-percutanea-fig3.jpg\" alt=\"occlusione percutanea\" class=\"wp-image-1026\"\/><figcaption class=\"wp-element-caption\">Fig. 3\nA, B: Occlusion of the left atrial appendage using a percutaneous device.\nC: An alternative, more recent method involves external access to the heart cavity (pericardial space) via a puncture below the sternum. The appendage is excluded from circulation using a lasso that externally constricts its neck.<\/figcaption><\/figure>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-kadence-column kadence-column191_5bf973-ef\"><div class=\"kt-inside-inner-col\">\n<p>In high-volume centers, the procedure has a success rate exceeding 95%,<\/p>\n\n\n\n<p>with a complication rate below 1%.<\/p>\n\n\n\n<p>The electrophysiologist selects the technique based on the patient's characteristics.<\/p>\n\n\n\n<p class=\"kt-adv-heading191_cea842-c4 wp-block-kadence-advancedheading\" data-kb-block=\"kb-adv-heading191_cea842-c4\">Dr. Grossi's Experience<\/p>\n\n\n\n<p><strong>Dr. Grossi has over 15 years of experience in percutaneous left atrial appendage occlusion and was the first in the world to develop and perform a combined procedure of endo-epicardial atrial fibrillation ablation and left atrial appendage occlusion using the Lariat system.<\/strong>&nbsp;For cases of atrial fibrillation resistant to standard ablation procedures, occlusion of the appendage using the Lariat lasso not only achieves hydraulic closure, eliminating the stroke risk, but also causes atrophy that eliminates its electrical activity, a key factor in maintaining atrial fibrillation.<\/p>\n<\/div><\/div>\n\n<\/div><\/div><\/div><\/div>\n\n<\/div><\/div>","protected":false},"excerpt":{"rendered":"<p>Che cosa \u00e8 l\u2019auricola sinistra?
